Upon request, a patient is reached by the doctor through the convenience of a phone or video consult. TriHealth Remote Patient Care: This program is by referral by your primary care provider only. Patients with specific health conditions can be referred into the remote patient monitoring program where they are assessed daily by a care team who communicates to the referring provider as needed (based on care protocols).
